The Practice runs electronic prescribing through the http://www.patient.co.uk/patient-access.asp website and all repeat prescriptions must be ordered in this way.
If you would like to order your prescriptions this way you need to register with us in person at reception. Please bring a form of photo identification with you.
If your Doctor agrees you may obtain your regular medications on repeat prescription. Attached to your prescription is a printout of your repeat medication.
To make your request:
- Click here and do it on-line
- Tick the items you require and place the list in the box at reception.
- Post your list to us and if you provide a stamped addressed envelope, we will post it back to you.
- Fax your printout to us on 01229 468217
PLEASE USE THE MEDICATION NAME LISTED ON THE RIGHT HAND SIDE OF YOUR PRESCRIPTION SHEET WHEN ORDERING A REPEAT ONLINE, USING THE DRUG NAME RATHER THAN THE BRAND NAME , UNLESS ADVISED NOT TO.
In the absence of a printout we require a written request (download one - see below - or we can provide you with a form at reception) ensuring your full details and details of the medication are provided.
To download the request form in MS Word click here, or in PDF format click here
(You can download Adobe Acrobat Reader for free if you don't already have it. Click on the link at the bottom of this page).
It is important to use the right hand side slip attached to your repeat prescription wherever possible as this is a clear and accurate list of your medications and it will avoid mistakes often made when using hand written requests
IF YOU USE YOUR COUNTERFOIL, PLEASE ALLOW 2 WORKING DAYS BEFORE COLLECTION.
ALL OTHER REQUESTS WILL TAKE 3 WORKING DAYS
WE REGRET WE ARE NOT ABLE TO ACCEPT TELEPHONE REQUESTS FOR PRESCRIPTIONS
