Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements.  This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.  There is further information about countries and vaccinations required on the links below.

PLEASE COMPLETE THE TRAVEL HEALTH QUESTIONNAIRE BELOW BEFORE COMING TO SEE THE NURSE

IF YOU ARE NOT REGISTERED WITH THE PRACTICE, YOU MUST INCLUDE YOUR HOME ADDRESS AND TELPHONE No ON THE FORM  - IN THE BOX MARKED 'EMAIL'

It is important to make this appointment as early as possible  - at least 4 weeks before you travel.  However, if this is not possible, still come, as vaccinations can be given right up to the day of departure, helping prevent serious illnesses while abroad.

Some vaccines incur a charge.  See below under Fees.

We are a Registered Yellow Fever Centre.

Further travel information can be found on the NaTHNaC website.
